A Rare Presentation of Muscle Involvement in Sarcoidosis: A Case Report

Background: Sarcoidosis can affect virtually any organ of the body, primarily lungs, lymphatic system, skin or eyes, a combination these sites and it is characterized by formation non-caseating granulomas. Muscular involvement in sarcoidosis usually asymptomatic pauci-symptomatic, its nodular form exceptional. Observation: In this study, we report case 36-year-old man who had been presenting with nodules both calves for about two years which onset was progressive. These were painless deeply embedded gastrocnemius muscles. There no functional complaints, general condition preserved. The physical examination, especially at level free other clinical signs except nodules. PET-scan (positron emission tomography) revealed few discretely hypermetabolic cervico-axillary lymph nodes multiple bilateral mediastino-hilar adenopathies involving all nodes, as well pulmonary foci. Biopsy accessory salivary glands (BASG) exhibited an epithelioid gigantocellular granuloma without necrosis that confirm diagnosis systemic sarcoidosis. search another location, notably cardiac, ophthalmological, neurological, renal, negative. respiratory function tests did not show restrictive syndrome abnormalities DLCO/VA ratio. It therefore mediastino-pulmonary stage II, diffuse node, splenic, muscle involvement. Conclusion: This shows importance BASG muscular sarcoidosis, impossibility performing biopsy.
